What to expect for your first appointment
Myomend Physiotherapy uses Jane App for booking and appointment management. Once your appointment is booked in, you will receive an email confirming your booking and an email prompting you to fill out an health history intake form. Please complete this form prior to your first appointment.
The address of the in-home clinic is 163 Taysham Crescent, Etobicoke, Ontario. The primary treatment area is located in the basement of the residence, and the gym area is located in the backyard of the residence.
Please ensure that you arrive on time for your appointment, as we cannot guarantee a full session time for late arrivals.
What to wear:
Please wear loose fitting clothing suitable for movement. Clothing like shorts/loose track pants and a t-shirt or tank top will your clinician to properly assess you without any restrictions.
Parking: There is free street parking available, or free parking available at Albion mall across the street. Please avoid parking in the driveway.
Physiotherapy Clients:
Upon your arrival, please text the following phone number: 647-520-0878. Feel free to wait inside your car, or enter the house from the side door, and wait in the waiting area until your designated appointment time.
Personal training Clients:
Upon your arrival, please call/ text the following phone number: 647-520-0878. Your clinician will come outside to escort you inside the gym space. If you do not receive an answer by phone, please knock on the side door and someone will come to escort you in the gym area.
Other:
Please note, a dog resides in the residence but will be in a separate location from the treatment area. Please let me know if you have any allergies to pets and we will do our best to accommodate.
Changing or Cancelling an appointment:
If you are no longer able to make your appointment, please contact through phone or text at: 647-520-0878. Please note that cancellations within 24 hours of your appointment are subject to a cancellation fee.
